Georgia St., Vancouver 5, . 685-7378 by Morton Mintz Washington special tons the former med ical director of a major drug company says the Job taught him the meaning loneliness and in an unusual 25-Page document the physician or. A. Dale console said flit after years i Check a Point where i could no longer live with myself. I had compromised to the Point where my Back was against a Wall and i had to choose Between re signing myself to total capitulation or resigning As medical finally on july 1, 1957, he left the firm e. R. Squibb and sons and entered the private practice of psychiatry in . Subsequently he testified about the drug Industry before the late sen. Justes Kefauver s Senate anti Trust subcommittee. Last March he was scheduled to testify before sen. Gaylord Nel s on s m o n o p o by sub committee but because of illness had to submit a written statement. Saturday the monopoly subcommittee released the 25-Page document. It consists of questions derived from the written testimony and of the 54-year-old physician s answers. Here Are excerpts on his resignation i Bear no malice toward Squibb. Actually i regret the need to implicate Squibb at All but if my testimony is to carry any weight i must choose Between mentioning them or remain ing silent i came to the belief that Squibb was if anything More ethical than most. To the question Why he resigned i believe that the Best answer can be found in my unfinished essay on the Good life of a drug company toward the end i said these Are Only some the things he must learn to rationalize. He must learn the Many ways to deceive the Fra food and drug administration and failing in this How to seduce manipulate or threaten the physician assigned to the new drug application for marketing into approving it even if it is incomplete. He must learn that any thing that helps to sell a drag is valid even if it is supported by the crudest testimonial while anything that decreases sales must be suppressed distorted and rejected because it is not absolutely conclusive proof. He must learn to word a warning statement so it will appear to be an inducement to use the drug rather than a warning of the dangers inherent in its use he will find himself squeezed Between business men who will sell anything to justify it on the basis that doctors ask for it and doctors who demand pro ducts they have been taught to want through the advert Ising and promotion schemes contrived by businessmen. If he can absorb All this and More and still maintain any sensibilities lie will learn the of loneliness and Aliena on doctors probably the greatest fault of the average practitioner is his inability to understand and to admit to the limits of Bis own competence. Unfortunately Many physicians enter into the fantasy entertained by their patients and fulfil their own infantile wishes to be Omni potent and omniscient. The average physician is not about to step off the throne that makes him god. A heavy prescriber is a physician who writes Over 100 prescriptions per week. This is the prime target of the detail Man Salesman and in this group we probably have the least discriminating physicians in the country. I find it difficult to believe that anyone can write that num Ber of prescriptions and still take time to discriminate. In this group we also have the physicians most Likely to raise a howl of protest Over the Prospect of taking away one of their toys. On drug testing i remember clearly an Occa Sion when we were making preliminary studies a drug that was being produced in very Small Quanti ties by a Laboratory opera Tion. We sent a highly placed authority enough of the drug to treat patients and were somewhat puzzled by the fact that he sent us favourable data on three patients. We Black listed him. To put the incident in its proper context i must confess that Black listing him consisted or taking his name out of the Hie of reliable investigators who could serve is adequate guides to important Deci Sions. His card was transferred to another file that indicated that he could be used As a proof Mill when. And if we should have need j of one. We did not report it to Fra and the secret was kept within the to the Best of my know ledge it still is. There were proof Mills that would deliver data at so much per head and in extreme cases. We used them. There were drugs that were declared useless after clinical trial by experts that subsequently be came marketable using the testimonials of less experienced physicians to prepare a new drug application for i have adequate reason to believe that other firms were less fastidious than we were and purchased favourable reports. On patients while it is shocking it is probably True that the chances that an average patient will get the right drug in the right amount at the right time is in the of 50 per cent the a Luckiest patient of All is the one who needs no drugs since if he has a complaint it is almost impossible to get out of the average practitioner s office without a prescription. On Selling to foreign countries the overseas divisions of Squibb held not Only the notion that the safety and Efficacy of a drug varied with the patient s National Ity but also that the advertising and promotion of drugs depended on the nationality of the physicians in any of the countries South of the Border were considered less sophisticated than . Physicians and a simpler approach had to be used. I recall one absurd proposal intended for the South american markets. It was proposed that we add to our inject Ible penicillin a sub stance that would produce the taste of garlic when it was absorbed. This it was said would impress the patient and would Lead him to anyone who believes in Placebo medication and the Mambo jumbo of the Art of Medicine. The proposal has no place in modern concepts of a science of therapeutics. The detail Man enthusiasm about the pro duct he is Selling plays an important role in How Many sales he makes. The members of my staff and i were Only a part of the manpower used to whip up enthusiasm Over a hum drum concoction. In Addi Tion innumerable prizes ranging from cutting boards to sets monogrammed glasses Are Given to those detail Man who reach or exceed a pro set quota sales. Since i was the confidant of Many of the detail men i Learned that Many of them had con Vinced or confused a Doc Tor to prescribe one of our products by telling the doctor that they were Only one step away from winning a prize. On research Many research programs were instituted not because they promised a worthwhile drug but because they promised profit in my opinion about 20 to 25 per cent of the total research and development budget Lovely Loos St. Albans England up cleaners who service Rural i Public lavatories in Hertford Shire have been offered one i week s pay As first prize in a Best kept toilet Competition.
