D OCTOR, I can't sleep. Can you
give me something?
That's a question I hear often,
and there's a reason
for it.
Insomnia is
common among
older adults. As
many as one- third
of adults and half
of those over age
65 report trouble
with sleep: Falling
asleep, waking up
too much in the
night, too early in
the morning, as well as feeling their
sleep was " non- restorative."
As we age, there are changes in
our sleep patterns, including more
frequent awakenings through the
night, and less time spent in the deeper
stages of sleep.
People who are tossing and turning
or who are up pacing all night with
insomnia often want a pill to help
them sleep. But taking pills can cause
trouble, especially for older people,
particularly those with health problems.
Sleep problems could be symptoms
of another health condition. Taking a
sleeping pill without knowing what's
causing the insomnia may mask the
symptoms of another illness, make it
worse, and delay appropriate treatment.
There are many causes of insomnia.
These include health conditions that
cause pain or shortness of breath,
neurologic conditions, dementia,
depression, anxiety and other psychological
disorders, and sleep disorders
such as sleep apnea and restless legs
syndrome. Some medications interfere
with sleep. People can also have
insomnia unrelated to other health
conditions, called " primary insomnia."
As a first step, talk to your doctor
or nurse practitioner. Some people
may need a referral to a specialist or a
sleep disorders clinic.
It would be wonderful to have a drug
that puts you to sleep quickly, keeps
you in a normal sleep all night, then
wears off completely after about seven
hours, without any side- effects. There
is, unfortunately, no such wonder drug.
Medications we do have for insomnia
include over- the- counter ( OTC)
drugs ( antihistamines or melatonin),
and prescription medicines including
sedative- hypnotics ( sleeping pills), sedating
antidepressants or antipsychotics,
and melatonin- like prescription
medications.
OTC sleep aids use the active ingredient
of a sedating antihistamine
( diphenhydramine; the medication
found in Gravol). Antihistamines are
used to treat allergy symptoms and
nausea, but can worsen memory and
thinking, as well as cause dry mouth,
constipation and urinary symptoms in
older adults.
The use of antihistamines as a sleep
aid has been poorly studied; we're not
sure if they work as a sleep aid in the
longer term, or if they're worth the
side effects.
Prescription medications include:
benzodiazepines - drug names ending
with "- azepam", e. g. lorazepam
( Ativan), diazepam ( Valium); Z- drugs
- zopiclone ( Imovane), zolpidem ( Ambien/
Sublinox); barbiturates - drug
names ending in "- barbital."
Barbiturates are not used these
days, as they have the most sideeffects.
Benzodiazepines and Z- drugs
also have many potential side- effects.
Sedating drugs can increase the
risk of falls in older people and worsen
memory and cognition. Regular, longterm
use of these drugs can lead to
tolerance, dependence or addiction.
Tolerance means your system " gets
used to" the drug, so the dose that
worked before no longer works, even
within a few weeks. Dependence occurs
after longer- term use, making it
difficult to reduce or stop the drug due
to " rebound insomnia." A few people
may become addicted to the drug,
seeking ever higher doses to the detriment
of their health.
Melatonin is used as a sleep aid,
but seems more helpful for jet lag
and shift work than for " standard"
insomnia. Ramelteon is a prescription
medication similar to melatonin,
which appears to have some benefits
for insomnia, but may cause daytime
sleepiness.
After this explanation, my patients
often say, " OK doc, I believe you.
There's no good sleeping pill. But I
can't sleep. What can I do?"
There is mounting evidence nondrug
treatments are effective in
improving sleep without risk of side
effects, and the benefits may last
longer than sleeping pills. This type
of treatment includes learning about
sleep hygiene, stimulus control, relaxation
techniques or formal cognitive behavioural
therapy, all of which change
our habits and program our bodies to
sleep when it's bedtime.
